OVERVIEW

Lack of follow up care after a hospitalization increases a patient’s likelihood of being readmitted. One potential partner for working with hospitals on reducing readmissions is the local county health department, who have resources to help patients obtain the necessary follow-up care. This program will highlight several initiatives with county health departments that can help reduce readmissions.
