Overview of Readmission Data in Florida
Florida was one of the first states in the country to publish “potentially preventable” readmission rates on the state’s consumer Web site, www.floridahealthfinder.gov, with readmission rates for certain conditions being publicly reported in 2008. Readmission rates are calculated using the "Potentially Preventable Readmissions" methodology developed by 3M Health Information Systems
- Readmission rates being studied in the collaborative are based on 15 day and 30 day readmissions to any acute care hospital in the state
- Readmissions are determined by linking patient social security numbers reported in the quarterly discharge data
- The PPRs are severity-adjusted using the 3M Health Information System’s APR-DRGs; and
With AHCA as a partner in the Collaborative, hospitals will not have to report additional data as part of this initiative. Instead, hospitals will be provided with reports that allow them to see more detail on their readmissions. Treo Solutions provides data support for the Collaborative.
