Objectives

The primary objectives of the Collaborative are:

1. To better understand why patients are being readmitted
2. Identify initiatives that reduce the likelihood of readmission
3. Share best practices in areas impacting readmissions
4. Identify policy and payment issues that impact readmission
5. Develop recommendations for consumer information for AHCA's consumer Web site
6. Evaluate the methodology used by AHCA to calculate readmissions
